Your experience matters.

You may submit your experience if you sought HBOT in Canada or as a Canadian, whether for a recognized indication, an emerging or non-recognized use, or by private or out-of-country means. CHR is interested in barriers of many kinds, including denial, delay, cost, geography, referral refusal, and forced travel.

Complete all sections — the form submits as a single step when you reach the end.

Examples of in-scope experiences

The registry welcomes submissions from Canadians who have encountered any of the following:

  • I was told I cannot receive HBOT because my condition is not on the official approved list.
  • I have an approved indication but was denied HBOT by a hospital or clinic.
  • No one in my province would refer me for HBOT despite my condition.
  • HBOT is technically available but only privately at a cost I cannot afford.
  • I learned there is no realistic access to HBOT for my condition in my region.

Privacy: All submissions are encrypted in transit, stored securely, and accessible only to authorized Registry staff. Your information will never be shared without your explicit consent. See our consent fields below for details.
